1. Service requests
   A person who wants to change the given name or surname, arrives onsite the registry office. At the time of service requests:
   1. subject to presentation of a personal identification document (original);
   2. shall submit a submission specifying the reason for the change the name, surname;
   3. minor parent or guardian and the name, surname of the consent of the Orphan's court - to be submitted to the original
   4. photograph 4,5x3,5 cm;
   5. a receipt of payment of state fee - original

3. Service Processing
   The Registrar, shall review and preparation for shipment documents submitted to the Ministry of Justice department of civil registries decision making.

4. Receipt of services
   Ministry of Justice department of civil registries decision regarding permission or refusal to change the nationality of the applicant shall be notified by mail registered letter.
   In the case of a positive decision applicant Office of Citizenship and Migration Affairs shall take the personal identification document.
